Healthfirst Spectrum Care Management

Ensuring you get the best care possible.

Your Healthfirst Medical Management team works with your primary care provider to improve your health and satisfaction.  Our programs help our members better manage their overall health and well-being, and to seek the care that they need in the healthcare system. Healthfirst offers care management services that bring added value to our members with chronic illnesses like hypertension, diabetes mellitus, asthma, congestive heart failure and behavioral health conditions.


Healthfirst Model of Care

All of the work that Healthfirst does to assess our members, to assign a dedicated case manager and to create an individual care plan approach is considered the Healthfirst Model of care.   The Healthfirst Spectrum of care programs are how Healthfirst cares for our members who need the greatest amount of help to stay healthy.

In particular, if you are a Healthfirst member with one of the following needs or conditions, there is a Healthfirst Spectrum Care Management clinical, outreach and educational programs that may help you:

Access Prevention services:

  • Ensure that you receive all of the preventive services that you need each year, including mammography, PAP testing, colorectal screening and care for older adults

Keep your Chronic Illnesses under control:

  • Diabetes Mellitus
  • Hypertension
  • Congestive Heart Failure
  • Asthma
  • Depression or Behavioral Health needs
  • Sickle Cell Disease
  • Cancer
  • COPD
  • Severe Neurologic Condition

Address Functional and Psychosocial needs that may impact health using a variety of community resources.


Interdisciplinary Care team:

Healthfirst creates and assigns an interdisciplinary care team to our members in care management, to help create a plan of care just right for you. Together we conduct health assessments and talk with you and your caregivers or designated representatives to determine the need for add-on services and benefits.  These needs are incorporated into the individualized care plan for each member.

Members of the interdisciplinary care team may include some or all of the following:

  • Primary care physician
  • Nurse practitioner, physician’s assistant, mid-level provider
  • Social worker, community resources specialist
  • Registered nurse
  • Restorative health specialist (physical, occupational, speech, recreation)
  • Behavioral and/or mental health specialist (psychiatrist, psychologist, drug or alcohol therapist)
  • Board-certified physician
  • Dietitian, nutritionist
  • Pharmacist, clinical pharmacist
  • Disease management specialist
  • Nurse educator
  • Pastoral specialists
  • Caregiver/family member
  • Preventive health/health promotion specialist
